Not everyone is a good candidate for outpatient tubal ligation reversal, so we ask you to start by:
Step 1- Filling out a new patient questionnaire will help us to get to know you. The questionnaire gives us details of your medical history, past surgeries, weight and height and other important details.
In general, outpatient tubal reversal surgery is successful in women less than 200 lbs. However, there is significant variation depending on your height and where fat is distributed. An inpatient tubal ligation reversal is also an option, but more expensive. We can let you know the details of that if you are not a good candidate for outpatient “same day” tubal reversal surgery.
Step 2- Obtain the operative report and pathology report from the doctor who did the tubal ligation procedure or by calling the hospital where it was performed.
They usually require you to complete a medical record release form in order to obtain a copy. If a copy of your operative report is not able to be found then, oftentimes, it is still possible to give you good advice about your chances of success. You can download the medical release form here.
Many women remember their doctor telling them how their tubes were tied. For instance, many women know that clips were applied. We are happy to help you with obtaining the operative report but you will still need to fill out a medical record release form and provide us with a copy.
Step 3- Complete the male medical release document so we can obtain your partners medical history. Also complete the new patient registration form.
Step 4- Fax or mail the questionnaire, the registration form, male medical release form, operative report, and pathology report to Jody Halloran. Our Fax number is 434-654-8521.
Step 5- Call Jody to acknowledge receipt and she will then set up a time for Dr. Williams, Bateman, or Smith to consult with you by phone if you live outside the region or an appointment here in the office.
Step 6- We always suggest a semen analysis be done prior to surgery. This can be done locally and we can order it for you.
Initial Evaluation by Phone
We want to make it as easy as possible for you toaccess us no matter where you live. You willneed a physical exam and consent for surgery with Dr. Williams , Dr. Smith, or Dr. Bateman prior to your surgery. If you live in the region, it is most efficient to have the initial visit and physical exam done here in advance. If you live more than 2 hours away, we are happy to contact you by phone to obtain your information and to complete much of the screening by phone. When your surgery is scheduled an appointment for a physical exam and to obtain your consent will occur the afternoon prior to surgery.
